Registering With The Practice

Anyone who lives within our practice area can register with the practice.
If you have chosen to register with our practice then please do so as soon as is convenient. Please personally bring into Reception two forms of ID.  One form being photo ID and another confirming your current address.  We will also require your NHS number for your registration.

Please do not wait until you are unwell as this will save you from having to complete the paperwork at a time when you may be feeling ill.

If you have a previous medical card this is helpful but not essential for the registration process. Once registered, your medical records are retrieved from your previous Doctor, this will ensure we are able to provide you with the best clinical care possible.

You will also be asked to complete a New Patient Registration Questionnaire before we can complete the registration process. Registration forms can either be collected from Reception or it is also possible to download a copy which will need to be completed by hand and returned to the Surgery along with the ID mentioned above. Once completed you will be registered with our practice and will be allocated a GP who will manage your care.
